Residents of Hammersmith and Fulham who have a long term medical condition are invited to take part in an event to help design better self-management practices on Wednesday November 19 from 9.30 am till 1pm at the St Paul’s Centre in Hammersmith.

The event run by North West London CCGs’ Patient Empowerment Team (PET), is aimed at people over 50 years old who have long term conditions like diabetes, dementia, chronic heart disease (CHD) and chronic obstructive pulmonary disease (COPD), their carers, voluntary and community groups, and health and social care professionals.

PET is looking at ways people with long term conditions and their carers can better manage their conditions away from hospital. After asking many patients and service users in our communitythe team says it is clear that many prefer to be treated at home.

Treating people in their homes, or other appropriate places where they feel comfortable also provides financial savings to the community as well as maximising the capacity of the whole health and social care system.

The workshop is part of the Whole Systems Integration programme which pools together existing budgets from health and social care to provide a more closely working Whole Systems approach to these services in local areas.

The workshop will be a World Café style event with participants asked for their views and experiences of what good Self –Management and Peer Support programme should be like.

Participants will be provided with lunch.

St Paul's Centre is part of St Paul's Church in Queen Caroline Street. It also hosts the Café on the Green, open Monday to Friday 8.30am – 4pm, serving ethically sourced coffee and fair-trade products as well as creating fresh food on site every day, using locally sourced produce wherever possible.
